About the Role

We're seeking an experienced and skilled Nurse Practitioner to join our team. In this role, you'll be responsible for providing direct patient care, assessing patient needs, developing and implementing individualized care plans, and collaborating with our multidisciplinary team to ensure seamless transitions of care.

Key Responsibilities
  • Provide comprehensive nursing care to patients, including medication administration, wound care, and other essential patient care activities
  • Conduct thorough assessments of patient needs, identifying opportunities for improvement and implementing evidence-based interventions
  • Develop and implement individualized care plans that address the unique needs and goals of each patient
  • Collaborate with our interdisciplinary team to ensure timely and effective communication, ensuring seamless transitions of care
  • Maintain accurate and detailed records of patient care, adhering to regulatory requirements and hospital policies
About You

To succeed in this role, you'll need:

  • A graduate degree in nursing from an accredited program
  • At least six months of experience as a Registered Nurse or equivalent in a comparable setting
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • A valid license to practice as a Registered Nurse in the State of California
  • Current certifications in BLS, ACLS, PALS, and TNCC
