Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Long Term Care Specialist. As a key member of our team, you will be responsible for reviewing and making recommendations on consumer assessments for long term care services and supports.

Key Responsibilities
  • Review and validate assessment outcomes to ensure accuracy and quality.
  • Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
  • Work collaboratively with colleagues to monitor and address clinical quality and gaps in care.
  • Provide clinical and educational assistance to support project initiatives.
  • Demonstrate exceptional communication skills necessary to interpret age-specific responses.
  • Participate in performance, improvement, and quality assurance activities and related meetings.
Requirements
  • Current NY Nurse Practitioner Licensure.
  • Proof of Board of Certification.
  • Proof of Malpractice Insurance showing professional liability coverage of 1M per incident/3M agg.
Preferred Qualifications
  • 2+ years practicing experience as a Nurse Practitioner.
  • Computer literacy and the ability to navigate new and emerging technology.
  • Must meet BYOD requirements.
  • Highly developed written and oral communication skills; strong interpersonal skills.
  • Ability to work in a fast-paced and changing environment with proven ability to work with community groups.
Compensation and Benefits
