Home Health Physical Therapist for Amedisys

Amedisys is seeking a highly skilled Home Health Physical Therapist to join our team. This is a 13 week contract, full-time position with a weekend rotation once a month.
Job SummaryAs a Home Health Physical Therapist, you will play a critical role in helping patients maintain, restore, and improve movement, activity, and function. You will work under a physician-approved plan of care to provide interventions, conduct reexaminations, modify interventions as needed, and perform discharge planning.
Your responsibilities will include making necessary revisions to the patient's plan of care in conjunction with the physician and documenting the patient's continual care needs. You will also provide physical therapy patient/client management, including interventions in accordance with the physician-approved plan of care.
You will be responsible for providing documentation of each visit according to the APTA Documentation Guidelines and accepted regulatory standards for inclusion in the patient's clinical record. Additionally, you will supervise Physical Therapy Assistants (PTA) in accordance with applicable municipal, state, and federal rules and regulations.
You will remain accessible by telecommunication, conduct regularly scheduled and documented conferences with the PTA, and make supervisory visits to the patient. You will also remain available upon request of the PTA for reexamination of the patient in instances of a noted change in the patient's medical status, response to interventions, or a discharge or discontinuation from physical therapy services.
You will include documentation of supervisory activities in the patient's medical record. You will participate in patient care conferences, in-services, chart audit activities, quality improvement activities, including OBQI and OBQM, and peer-review activities.
You will make recommendations for follow-up care in coordination with the care center that admitted the patient and utilize other care center care providers to assist with referrals to community resources. You will also supervise the Home Health Aide when SN is not active in the case, communicate plan of care changes to the Aide and other appropriate staff as necessary.
You will keep abreast of physical therapy trends and knowledge for the service provision, documentation, and care coordination. You will comply with policies, procedures, and regulatory mandates, including but not limited to abiding to the terms of the Amedisys Compliance Program.
We are looking for a candidate with a current, unencumbered license to practice physical therapy specific to that state they are assigned to work by the Company. A graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE) is also required.
A minimum of one year of Home Health physical therapy experience is preferred. CompensationThe estimated salary for this position is $60,000 - $80,000 per year, depending on location and experience.
