Care Navigator

About the Role:
We are seeking a skilled Care Navigator to join our team at Sunflower Health Plan. As a Care Navigator, you will play a critical role in developing and implementing care management activities to provide quality, cost-effective healthcare outcomes for our members.
Key Responsibilities:
- Evaluate member needs, barriers to care, and resources available to recommend and facilitate the best outcome
- Develop and maintain personalized care plans, identifying providers, specialists, and community resources needed for care
- Provide psychosocial and resource support to members, caregivers, and care managers to access local resources and services
- Coordinate between members, caregivers, and care providers to ensure timely access to care and services
- Monitor progress towards care plan goals and collaborate with healthcare providers for revisions or addressing identified member needs
- Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third-party payer regulators
Requirements:
- Bachelor's degree and 2-4 years of related experience
- Current state clinical license preferred
