Pharmacy Care Coordinator

The Pharmacy Care Coordinator plays a vital role in helping clients navigate the healthcare system, empowering them with the information and insight they need to make informed decisions about their health and well-being.
This position requires a strong understanding of pharmaceuticals, clinical guidelines, and patient education. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with internal and external teams.
Key Responsibilities:
- Analyze client data to identify opportunities for intervention and develop targeted strategies to improve health outcomes.
- Work closely with care teams to execute therapeutic plans, ensuring the most effective and economically viable medication treatment options are used.
- Provide patient education and support, including oral education on medications and evaluation of patients' and caregivers' ability to understand medication instructions.
- Utilize multiple data analytics programs to identify clinical opportunities and improve clinical and financial outcomes.
- Act as an onsite Benefits Champion, participating in monthly benefits training and proactively creating a culture of personal health and responsibility.
Requirements:
- Current Certified Pharmacy Tech (CPht) License or at least 3 years of clinical experience with pharmaceuticals.
- At least 2 years of general pharmacy experience, with a preference for experience in managed care, the PBM industry, or pharmaceutical sales.
- Ability to work on multiple tasks, prioritize, resolve complex issues, and have strong project management skills.
- Proficiency in using Microsoft Office applications and ability to talk and type at the same time, with multiple tabs/programs open.
This is a remote position, and the Pharmacy Care Coordinator must reside in California. The ideal candidate will be able to work independently, as well as collaboratively with internal and external teams to ensure clients and their members receive superior service.
