Homecare Registered Nurse Opportunity

We are seeking a skilled and compassionate Registered Nurse to join our Homecare team at Hospice of Northwest Ohio. As a Homecare Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare team members
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs
  • Administer medications and treatments as ordered by physicians, and document patient responses and outcomes
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
Requirements:
  • Graduate of an accredited school of registered nursing; BSN preferred
  • Currently licensed as an RN through the Ohio Board of Nursing; must be able to obtain a license in the state of Michigan
  • Minimum 2 years of medical-surgical nursing experience preferred; previous experience in homecare, and/or hospice experience is preferred
  • Has working experience charting in an EMR system; Netsmart system preferred
  • Possesses clinical expertise related to critical thinking, assessment, problem-solving, and technical skills
  • Possesses the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of intervention
  • Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues
  • Has the ability to work independently and within an interdisciplinary team
  • Demonstrates strong organizational, time management, and prioritization skills
  • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements
  • Must be CPR certified according to the American Heart Association CPR standards
  • Understands the hospice philosophy, principles of death/dying, concepts of pain/symptom management, and adult teaching/learning principles
