About the Job:

The Therapist I is a key member of the multidisciplinary team responsible for delivering direct clinical services to patients and their families. The primary focus is on completing comprehensive psychosocial assessments, providing individual, group, and family therapy sessions, facilitating psycho-educational training, family meetings, and discharge planning. Additionally, the Therapist I assists in program development, develops and implements treatment plans, and provides linkage to outside resources.

Roles and Responsibilities:

  • Conducts a thorough psychosocial assessment and develops an initial discharge plan.
  • Develops and implements treatment plans for patients based on assessment and coordinates any additional services needed, revising as necessary.
  • Conducts individual, group, or family sessions as appropriate for the treatment plan of the patient.
  • Applies psychotherapeutic techniques and interventions in the delivery of services to individuals and families for the purpose of treating diagnosed emotional and behavioral disorders.
  • Participates in multi-disciplinary treatment planning conferences, presenting patient information to the treatment team and recommending effective treatment interventions.
  • Maintains a caseload as assigned.
  • Develops and implements special patient groups or program modules.
  • Timely prepares progress notes and treatment updates in the medical record, maintaining all clinical documentation in accordance with regulatory and accrediting standards.
  • Ensures each assigned patient has a structured and clinically appropriate post-hospitalization care plan, acting as a liaison to referral sources and supporting continuation of care.
  • Provides crisis intervention to patients and families in acute distress.
  • Provides introduction and initial coordination of a wide range of community resources, including financial, housing, legal, social/recreational, occupational, spiritual, and support groups.
  • May perform case management, discharge, or aftercare planning as needed.
  • Requires a Master's Degree in Social Work, Counseling, or Psychology and at least one year of experience as a therapist with all age groups.
