As an Advanced Caregiver Specialist, you will play a vital role in providing high-quality patient care in the comfort of their own homes. Your expertise will be essential in performing technical skilled care, working within the scope of practice as defined by state laws, and collaborating with the healthcare team to achieve optimal patient outcomes.

Key Responsibilities:

  • Providing quality patient care and performing technical skilled care in the patient's home
  • Working within the scope of practice as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other members of the healthcare team
  • Instructing patients' families in the care of the patient and maintenance of a healthy environment for the patient
  • Observing, recording, and reporting to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily

Requirements:

  • Current LPN licensure in the state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • CPR Certification required
