Nurse Practitioner Diabetes Specialist

This is a 40-hour position that may require weekend and holiday work. The Diabetes Advance Practice Provider (APP) is a registered nurse or physician assistant with specialized knowledge and skills in diabetes management.
Key Responsibilities- Assess and manage patients with diabetes, including diagnostic and therapeutic interventions.
- Develop and implement individualized plans of care for patients with diabetes.
- Collaborate with attending physicians, interdisciplinary teams, and other healthcare professionals to provide comprehensive care.
- Provide education and support to patients and their families on diabetes management and self-care.
- Participate in quality improvement initiatives and contribute to the development of diabetes care policies and procedures.
- Graduate degree in a Physician Assistant program or a Master's degree in Nursing with a focus on Nurse Practitioner education.
- Current licensure as a Physician Assistant in Massachusetts or a Massachusetts Nurse Practitioner License.
- Board Certification by the National Commission on Certification of PAs (NCCPA) or the American Nurses Credentialing Center (ANCC).
- Experience in inpatient diabetes management and a strong understanding of hospital-based and community-based care.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Certified Diabetes Care and Education Specialist (CDCES) or Board Certification in Advanced Diabetes Management (BCADM).
- Experience with electronic health records and other healthcare information systems.
- Knowledge of and commitment to the principles of team-based care and awareness of local, regional, and national trends in chronic disease care redesign and transformation.
The APP position on the team can be challenging, requiring both teamwork and individual initiative. The work schedule may be subject to change based on patient care needs.
