Key Responsibilities
  • Provide high-quality patient care in their homes as prescribed by physicians and in compliance with applicable laws and regulations.
  • Coordinate total care plans with other healthcare professionals, ensuring continuity of patient care through effective communication and collaboration.

You will make initial and comprehensive nursing evaluations, determine primary care focuses, and develop accurate plans of care within state-specific guidelines. Additionally, you will ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.

Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current drivers' license, vehicle insurance, and access to a dependable vehicle or public transportation
Compensation

The estimated salary for this role is approximately $65,000 - $80,000 per year, depending on experience and qualifications. This compensation package reflects our commitment to attracting top talent and providing a competitive benefit structure.
