Occupational Therapist Job Summary:

As a Home Health Occupational Therapist with San Diego Home Health Care, you will play a vital role in helping patients recover, improve, and maintain skills needed for daily living. Your expertise will be invaluable in evaluating and treating patients with injuries, illnesses, or disabilities in their homes.

Key Responsibilities:
  • Evaluate patient functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity.
  • Evaluate the home environment for hazards or barriers to more independent living.
  • Identify equipment needs and provide supervision to Occupational Therapy Assistants.
  • Conduct initial evaluations/assessments and report findings to the Health Care Provider and RN, developing a plan of care promptly for patients.
  • Document initial evaluations and treatment plans, as well as re-evaluations of patient progress at least every thirty (30) days, updating the plan of care as necessary.
  • Complete comprehensive assessments and OASIS data on a timely basis.
  • Observe, record, and report to the Health Care Provider the patient's treatment, response, and progress, promptly reporting significant changes.
  • Obtain orders to revise the plan of care.
  • Instruct patients and caregivers in the use of alternate methods and devices for upgrading independence in activities of daily living.

Qualifications:
  • Bachelor's Degree and active Occupational Therapist license in the state of practice.
  • At least one (1) year of work experience under a qualified Occupational Therapist, with home health experience preferred.
  • A valid driver's license and automobile insurance.
  • Ability to make home visits and provide occupational therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner.
  • Home health and Home Care Home Base documentation experience.
