Job Summary

The Clinical Discharge Coordinator - Patient Advocate plays a pivotal role in ensuring seamless transitions for patients from hospital to post-acute care settings. This role involves a multifaceted approach, encompassing patient advocacy, care coordination, and discharge planning to facilitate timely and effective care transitions.

Key Responsibilities
  1. Collaborative Patient Care Planning

The Clinical Discharge Coordinator - Patient Advocate will work closely with patients, families, and healthcare teams to develop and implement individualized care plans, taking into account patients' medical, rehabilitative, psychosocial, and physical needs.

Discharge Planning and Coordination

Responsible for coordinating all aspects of discharge planning, including arranging post-acute care services, securing necessary equipment, and ensuring patients receive necessary education and support for smooth transitions.

Care Transition Management

Monitors patients' progress, identifies potential barriers to discharge, and works with healthcare teams to address these issues, ensuring timely and effective care transitions.

Patient Advocacy

Serves as a patient advocate, empowering patients to make informed decisions about their care, and assisting them in navigating complex healthcare systems.

Requirements and Qualifications
  • Education: Associate Degree in Nursing (ASN) or BA/BS in Social Work
  • Experience: Minimum 2 years' experience in utilization review and discharge planning
  • Licensure/Certification: Registered Nurse, Licensed Social Worker, Clinical Social Worker, or LMSW as appropriate based on educational preparation
Estimated Salary Range:

$65,000 - $85,000 per annum, depending on experience and qualifications.
