Job Title: Medical Care Coordinator

Job Summary:

We are seeking a skilled Medical Care Coordinator to join our team at Addiction and Mental Health Services, LLC. As a Medical Care Coordinator, you will play a vital role in coordinating and delivering quality medical care to our patients.

Responsibilities:

  • Schedule new and follow-up patient appointments
  • Monitor and review patient schedules for next appointments
  • Provide patient appointment reminders
  • Coordinate provider schedules
  • Monitor medications and document weekly medication count and changes
  • Prepare patients for examination and treatment, including taking vital signs
  • Communicate and document providers orders/recommendations
  • Obtain necessary patient information to meet current Federal guidelines for electronic medical records
  • Identify and resolve minor patient billing complaints
  • Assist in ordering, receiving, and stocking of medications and medical supplies
  • Maintain cleanliness of medication closets

Requirements:

  • LPN required
  • CPR certified or within 30 days of hire
  • Clinical experience required: LPN
  • Experience in Substance Abuse Treatment and/or Medication Assisted Treatment, preferred
