Salary: The estimated salary for this position is $83,000 - $113,000 per year, based on experience and qualifications.
Responsibilities: As a Healthcare Transition Coordinator, you will be responsible for facilitating the transition of patients from one level of care to another. This will involve collaborating with multidisciplinary teams, assessing patient needs, and developing personalized transition plans. You will also participate in Utilization Review and Patient Advocacy efforts.
Requirements: To be successful in this role, you will need to have a high degree of skill in written and oral communications, as well as personal interactions. You must be able to demonstrate agility in a fast-paced environment and possess computer competency. A minimum of three years of clinical experience in a healthcare delivery system is required, along with completion of an accredited Nursing program and current Nursing License in the State of MA.
Preferred Qualifications: Experience in Case Management, Home Care Discharge Planning, and utilization of InterQual would be highly valued. Good critical thinking skills and willingness to learn in a fast-paced environment are essential.
