About the Role

We are seeking a skilled Community Health Nurse to join our Home Care team at Rochester Regional Health. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and education to individuals in their own homes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' bio-psycho-social needs and develop individualized care plans
  • Provide skilled nursing care and prescribed treatments to patients based on physician orders
  • Deliver patient and family education in accordance with patient assessment and care plan
  • Maintain effective communication with office and supervisory staff, as well as coordinate client services and referrals
Requirements
  • Associate's Degree in Nursing (AS) required; Bachelor's Degree in Nursing (BSN) preferred
  • 1 year of nursing experience preferred, with prior home health, clinical, and direct patient care experience
  • Ability to work independently and as part of a team
