AIM Independent Living Center seeks a skilled Health Home Care Manager to join our team. This is an exciting opportunity to make a meaningful difference in the lives of our patients and their families.

We are seeking a highly motivated and organized individual with strong communication skills to manage patient care and services. As a Health Home Care Manager, you will be responsible for conducting comprehensive assessments, providing education and support, and collaborating with community providers to ensure seamless care delivery.

• Conduct thorough medical, behavioral health, and social service needs assessments for all assigned health home enrollees.
• Develop and implement personalized care plans that address patients' unique needs and goals.
• Collaborate with various community providers and resources to obtain necessary services and support.
• Document patient progress, activities, and other required information.

• Bachelor's degree or equivalent experience in a related field.
• At least two years of relevant experience in healthcare management or a related field.
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
• Sound computer knowledge and skills, including proficiency in health information technology.
• Valid NYS driver's license and reliable transportation.

The estimated salary range for this position is $55,000 - $65,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as opportunities for professional growth and development.
