WellSpace Health seeks a skilled Case Manager II Specialist to support the delivery of comprehensive care to our community.

About the Role

This is a challenging and rewarding opportunity to join our team as we strive to provide high-quality healthcare services to underserved populations.

The successful candidate will be responsible for providing case management services, including intake needs assessments, advocacy, and linkage to community resources. They will work closely with patients to develop SMART treatment plans and ensure seamless coordination of care.

We are seeking an experienced professional with strong communication skills, a passion for delivering exceptional patient-centered care, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Conduct individualized intake needs assessments to identify patient goals and priorities
  • Represent and/or advocate for patients in obtaining identification, housing, benefits services, and entitlements from governmental and private agencies and organizations
  • Support patients with completing HMIS, housing plans, housing resources, and housing applications
  • Collaborate with patients to develop SMART treatment plans that address co-occurring disorders (mental health and substance abuse)
  • Write and enter progress notes daily into EHR systems to maintain patient files
Requirements
  • High School Diploma/GED and 12 months Case Management experience or relevant education and certification preferred
  • Demonstrated ability to work collaboratively with others in a team environment, respecting and valuing the perspectives and experiences of others
  • Twelve (12) months of experience with Harm Reduction, Motivational Interviewing, Patient Centered theory, Cultural Humility, in addition to Substance Abuse issues and Treatment
  • Able to function effectively in a fast-paced environment and maintain accurate records for reporting to various funding and inspecting agencies
  • Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment
