Our teams collaborate with existing treatment providers and link clients to additional services offered by Sheppard Pratt or community providers.
Key Responsibilities:
- Provide a safe and therapeutic environment encouraging independence and well-being.
- Work one-on-one with clients to learn or regain skills for successful community and independent living and socialization.
- Ensure medication compliance, including safety, dosage, and cleanliness.
- Participate in a collaborative service team to develop and implement individual service plans.
- Provide transportation and attend clients' medical appointments, offering advocacy services throughout.
