About the Role
We're looking for a dedicated and compassionate LPN Nurse Care Coordinator to provide high-quality patient care and support our Registered Nurses. As a key member of our healthcare team, you'll be responsible for coordinating patient care, prioritizing tasks, and implementing interventions to achieve optimal patient outcomes.

Required Skills and Qualifications
To be successful in this role, you'll need:
* A current State LPN License
* High School Diploma or GED and LPN diploma and Licensure
* Minimum experience: New graduate LPNs or those with less than one year of experience will be referred to the LPN residency program

Salary Range
$45,000 - $65,000 per year, based on experience and location.

Job Responsibilities
Your responsibilities will include:
* Patient Assessment: Assist with data collection and provide feedback to the Registered Nurse from various sources to enhance clinical decisions about treatments and nursing interventions.
* Planning: Provide feedback to the Registered Nurse in the development of nursing diagnoses and carry out the established plan of care and document reassessments, interventions, and patient responses to interventions.
* Implementation: Carry out therapeutic interventions established in the interdisciplinary plan of care, implement interventions with safety, skill, and efficiency, and adjust according to patient response. Document patient response to interventions according to hospital standards.
* Evaluation: Provide real-time feedback to the Registered Nurse in the evaluation of interventions and treatments and how they affect patient health status and expected outcomes. Evaluate patient responses to nursing care and therapies. Document responses to nursing interventions according to hospital standards.

Quality of Care
You'll be responsible for delivering high-quality patient care that's non-judgmental, sensitive, and culturally competent. You'll develop a therapeutic relationship with patients that upholds humane/holistic healthcare congruent with the patient's needs, beliefs, and values. You'll adhere to established parameters for medication administration and participate in quality improvement activities to improve clinical care.

Education and Professional Development
You'll participate in educational activities to improve clinical knowledge and enhance role performance. You'll complete annual competencies as indicated and maintain records that provide evidence of competency and acquired knowledge, skills, and certification necessary for practice setting.
