About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team in Olympia. As a LPN, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Document patient progress and communicate with healthcare teams to ensure continuity of care.
Requirements
  • Graduate of a school of nursing.
  • Washington Licensed Practical Nurse upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • National Provider Identifier (NPI) number within 30 days of hire.
  • 1 year Nursing experience in an acute care setting.
Preferred Qualifications
  • 1-year Home health care experience.
