This position offers a unique opportunity to work as a Home Health Physical Therapist Assistant with All Medical Personnel. As a key member of our interdisciplinary team, you will collaborate with patients, families, and healthcare providers to develop holistic plans of care that focus on achieving patient-centered goals.

Responsibilities:

  • You will instruct, coach, guide, and intervene to support patients and families in achieving optimal health outcomes while remaining in their communities.
  • Promote optimal health, well-being, and quality of life for home health patients and their families by providing skilled, intermittent physical therapy in the patient's home.
  • Practice independently in the patient's home using excellent, evidence-based practice for assessment, clinical decision-making, clinical practice, and coordination of care across the continuum.
  • Act as case manager and team lead for patients requiring home health therapy services when nursing is either not involved in the case or physical therapy is the primary patient need.
  • Assess patients and collaborate with the interdisciplinary team and primary care provider to develop patient-centered plans of care.
  • Provide physical therapy treatment to patients/families/caregivers as ordered.
  • Complete accurate clinical documentation to effectively support service provided within specified timeframes.
  • Provide physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth and development and life stages.
  • Maintain communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
  • Report acute medical problems and condition changes to the primary care provider.
  • Supervise Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.
  • Complete regulatory required assessments within specified timeframes.
  • Complete medication reconciliation at specified timeframes.
