Job Summary:

We are seeking an experienced Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will provide holistic care to patients across the health continuum, utilizing the nursing process to assess, diagnose, plan, implement, and evaluate patient outcomes.

Salary:

The salary for this position is estimated to be between $32.57 and $55.09 per hour, based on experience and qualifications.

Responsibilities:

  • Advocate for patients and their families, ensuring that their needs are met and their voices are heard.
  • Utilize the nursing process to deliver high-quality, evidence-based care, taking into account the individual needs of each patient.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans, prioritizing patient-centered outcomes.
  • Monitor patient progress, reassessing and adjusting care plans as needed to ensure optimal outcomes.
  • Demonstrate expertise in acute-care nursing, leveraging knowledge and skills to drive positive patient experiences.
  • Ensure compliance with regulatory requirements, standards of practice, and professional scope of practice.
  • Participate in ongoing education and training to enhance clinical knowledge and skills, staying current with best practices and emerging trends.

Requirements:

  • Bachelor's degree in Nursing (BSN) or equivalent experience.
  • Licensed in the State of Vermont.
  • BLS Certification through American Heart Association.
  • Unit-specific certifications as required (ACLS, TNCC, PALS, ENPC) per specific department operational framework.
  • Completion of endorsed patient/visitor de-escalation and restraint program certification based on unit specific guidelines.

Preferred Qualifications:

  • Prior related nursing experience, preferably in acute-care settings.
  • Demonstrated proficiency in basic computer skills.
