About the Role

We are seeking a highly skilled and compassionate Care Manager to join our team at MidPen Housing. As a key member of our Resident Services team, you will play a vital role in supporting our residents in achieving stability and self-sufficiency in their housing.

Your primary responsibility will be to engage with residents, provide case management services, and connect them with community resources to address their unique needs. You will work closely with our Property Management team and other stakeholders to ensure seamless service delivery and effective communication.

As a Care Manager, you will be responsible for:

  • Conducting regular outreach and engagement with residents to understand their needs and concerns
  • Providing case management services, including goal setting, service planning, and follow-up
  • Linking residents to community resources, such as healthcare services, employment assistance, and educational programs
  • Collaborating with local service providers to ensure comprehensive support for residents
  • Participating in staff meetings, trainings, and other site-sponsored events to stay informed and engaged

To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work effectively with diverse populations. You will also need to be comfortable using technology, including our Salesforce system, to track resident interactions and service delivery.
