Job Summary

The Clinical Care Coordinator RN plays a vital role in providing professional nursing care through skillful assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. This position requires strong communication and organizational skills, as well as the ability to maintain a safe work environment and perform all related job responsibilities in a safe manner.

Key Responsibilities
  • Clinical judgment and critical thinking skills to prioritize patient needs and develop effective care plans.
  • Strong communication and interpersonal skills to collaborate with patients, families, and healthcare teams.
  • Ability to maintain accurate and detailed records, including medical histories, treatment plans, and progress notes.
  • Physical stamina to perform job requirements, including standing, walking, and lifting up to 35 pounds.
Requirements

To be successful in this role, you will need:

  • A graduate degree from an accredited School of Nursing.
  • BLS certification per policy guidelines.
  • A current RN license or temporary license as a Registered Nurse Petitioner in the state where you work and reside.
