Job Summary:
We are currently seeking a compassionate and skilled Caregiver - Home Health Care Specialist to join our team. In this role, you will have the opportunity to work directly with clients in their homes, providing personalized care and support to help them achieve their goals and live independently.
About the Job:
This is a full-time or part-time position that requires a high level of flexibility and adaptability. You will be working closely with clients and their families to develop and implement individualized care plans, as well as collaborating with other members of the care team to ensure seamless coordination of services.
Key Responsibilities:
* Provide direct care and support to clients in their homes, including assistance with daily living activities, mobility, and medication management
* Develop and implement individualized care plans in collaboration with clients and their families
* Work closely with other members of the care team to ensure seamless coordination of services
* Maintain accurate records and documentation of client care and services provided
Requirements:
* High school diploma or equivalent required; 1 year of experience working with disabilities preferred
* Must have a valid driver's license and reliable transportation; proof of insurance required
* Ability to lift up to 50 pounds and perform physical demands of the job
* Excellent communication and interpersonal skills; ability to work effectively with clients, families, and care team members
