We are seeking a highly skilled and compassionate Nurse Care Coordinator to join our team at St. Vincent's Medical Center in Bridgeport, CT.

About the Role:

This is a full-time position with a competitive salary range of $70,000 - $90,000 per year, depending on experience. As a Nurse Care Coordinator, you will play a vital role in ensuring that our patients receive the highest quality care and support throughout their hospital stay and beyond.

Job Description:
  • Provide direct nursing care to patients in accordance with established policies and procedures.
  • Implement and monitor patient care plans, ensuring seamless communication among healthcare teams.
  • Serve as a primary coordinator for all disciplines, guaranteeing well-coordinated patient care.
  • Assess and coordinate patient discharge planning needs, collaborating with the healthcare team to ensure smooth transitions.
Required Skills and Qualifications:
  • Graduate of an accredited nursing school.
  • Current Connecticut RN license.
  • BLS certification by the American Heart Association.
  • Associate degree required; BSN preferred.
  • Two years of Medical-Surgical/Critical Care experience preferred.
