About the Position

The ideal candidate will have a strong educational background in family health, preferably with a Master's degree from an accredited nursing program. You will be certified as a Family Nurse Practitioner by a recognized organization and possess excellent communication skills, both written and verbal.

Key Responsibilities
  1. Deliver comprehensive medical assessments, including history and physical exams, to identify gaps in care and preventative needs.
  2. Develop and implement treatment plans, collaborate with specialists as needed, and prescribe medications as appropriate.
  3. Schedule patient appointments, provide post-discharge coordination, and perform face-to-face visits in various settings.
  4. Participate in regional meetings, contribute to best practices development, and engage in ongoing education and training to enhance clinical knowledge and skills.
Requirements
  • Master's degree in family health from an accredited nursing program.
  • Current state-issued license to practice as a Family Nurse Practitioner.
  • National certification from a recognized organization (e.g., American Academy of Nurse Practitioners; American Nurses Credentialing Center).
  • Current Basic Life Support for Healthcare Professional certification.
  • Valid driver's license.
