Senior Patient Care Coordinator

As a Senior Patient Care Coordinator with Providence Regional Medical Center Everett, you will play a vital role in supporting the Social Work team in the Care Management Department. Your primary responsibility will be to perform a variety of patient-centered administrative tasks to ensure seamless care coordination and discharge planning.
Key Responsibilities
- Work under the direction of the Social Work team to support patients and families with their specific discharge plan and plan of care.
- Provide administrative support to professionals, including customer service and insurance payer experience.
- Collaborate with healthcare teams to ensure accurate and timely communication.
- Develop and maintain accurate patient records and reports.
Requirements
- Associate's Degree or equivalent education/experience.
- Coursework/training in medical terminology.
- Demonstrated work experience with patients and families.
- Work experience in a healthcare setting, preferably providing customer service and administrative support.
Preferred Qualifications
- Coursework/training in healthcare.
- MS Office Certification upon hire.
- MS Word and Excel software programs.
