Job Title: Patient Navigator Job Summary: We are seeking a highly skilled Patient Navigator to join our team at the University of Oklahoma. The successful candidate will be responsible for assisting with daily functions, patient outreach, and identification of community resources for patients. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. Key Responsibilities: * Provide patient counseling and referrals for patient needs * Assist with coordinating patient care * Manage patient continuity of care for patients entering and going through the multi-disciplinary clinics * Organize and conduct community outreach on behalf of the Program * Maintain a customer service program to ensure high-quality service and customer satisfaction for patients in the clinical setting * Create and conduct surveys for evaluating satisfaction of the users of the program * Participate in and initiate advancement of hospital, community, and patient relations * Survey and follow-up with referring providers to ensure high-quality customer service * Meet with patients to ensure customer satisfaction with the Program and hospital experience * Utilize and/or create databases for tracking and following patients through outpatient clinical settings * Organize and analyze program data on a regular basis * Create and maintain referring physician informational databases * Maintain all documentation and files on behalf of the program Requirements: * Bachelor's Degree in Health Education, Social Work, Psychology, Nursing, or health-related field * 12 months experience in a medical or clinical care setting Preferred Qualifications: * Bilingual - English/Spanish Why You Belong at the University of Oklahoma: The University of Oklahoma fosters an inclusive culture of respect and civility, belonging, and access, which are essential to our collective pursuit of excellence and our determination to change lives.
