Job Overview

The Support Worker Coordinator plays a crucial role in delivering exceptional quality of service and providing engaging activities in a safe and healthy environment.

Key Responsibilities:
  • Assist clients with basic living skills and daily personal tasks, including but not limited to showers, dressing, housekeeping, grocery shopping, meal preparation, and financial planning.
  • Provide reliable transportation to and from activities and appointments as needed, requiring a valid Arkansas driver's license.
  • Maintain effective communication via various means, such as the facility logbook, staff meetings, and updates on client needs and progress towards their plan of care.
  • Accurately complete all required documentation, including Behavioral Reports, Incident Reports, Compliance Forms, Personal Care Work Sheets, Requisitions, Maintenance Forms, Progress Reports, Monthly Attendance Logs, and other necessary records.
  • Ensure strict adherence to and completion of necessary documentation and maintenance of records for FTA vehicles, maintaining compliance with Medicaid guidelines.
  • Immediately report incidents and/or emergencies to supervisors or designated authorities, as this is a safety-sensitive position.
  • Assist in formulating, managing, revising, and following through on clients' Plans of Care to ensure timely treatment and prioritize good health, safety, and security.
  • Adhere to proper medication management guidelines and assist in monitoring and documenting these procedures.
  • Organize and conduct recreational activities for clients.
  • Foster a positive example for clients through suitable physical appearance, adherence to FCC's dress code, and responsible actions.
  • Treat clients and their families with dignity and respect at all times, representing FCC's mission in all aspects.
  • Contribute to maintaining FCC facilities and grounds, ensuring cleanliness, orderliness, and functionality.
  • Participate in vehicle maintenance, ensuring cleanliness and regular preventive maintenance.
  • Attend necessary training and continuing education sessions.
  • Possess liability insurance and maintain current coverage.
  • May be required to undergo periodic driving tests and demonstrate proficiency in operating a motor vehicle, including passenger vans. Additionally, employees may be required to drive their personal vehicles at times and maintain adequate liability coverage on any personal vehicle used for transporting participants.
  • Guarantee confidentiality of all information received regarding Friendship Community Care, Senior Center Programs, and clients, adhering to Friendship Community Care's Non-Disclosure Policy and HIPAA Policies.
