About the Role

San Ysidro Health Center is seeking a highly skilled Licensed Clinical Social Worker (LCSW) to join our team as a Behavioral Health Specialist. This is an exciting opportunity to work in a dynamic and patient-centered environment, providing high-quality behavioral health services to individuals and families in our community.

Key Responsibilities
  • Evaluate and provide brief psychological/behavioral health interventions to patients with physical health, mental health, and/or substance abuse diagnoses.
  • Provide consultant services to clinical staff/primary care providers based on the Primary Care Behavioral Health Consultation model.
  • Perform brief assessments of patient needs and develop negotiated care plans with patients and primary care clinicians.
  • Maintain accurate and timely documentation in electronic medical records and produce reports of evaluation and/or treatment as required.
  • Evaluate and/or revise care plans as indicated, with appropriate documentation in progress notes and communication with relevant multi-disciplinary team members.
  • Perform crisis interventions as needed, including conducting suicide assessments and making recommendations to medical providers and patients.
  • Work in coordination with the Chief Behavioral Health Officer/Director of Behavioral Health to provide information to healthcare providers about mental illness in the medical setting.
  • Assist in the implementation and administration of the Enhanced Screening, Brief Intervention, and Referral to Treatment (ESBIRT) program model for substance abuse prevention and/or treatment.
  • Participate in meetings as required, including weekly multi-disciplinary/case conference/pod care team meetings with medical, clinical, and intern staff.
Requirements
  • Sensitivity to multi-cultural concerns and the psychotherapeutic process.
  • 2 years of experience in a community health/mental health setting preferred.
  • Experience in Behavioral Medicine/Health Psychology preferred.
  • Master's degree in Social Work from an accredited school or its equivalent from an accredited school.
  • LCSW issued by the state of California and a valid driver's license and access to an insured vehicle required.
  • Bilingual English/Spanish required, with excellent oral and written communication skills.
