RN Case Manager Job Opportunity

Job Summary:

A highly skilled RN Case Manager is required to provide comprehensive care to patients with serious illnesses. The ideal candidate will assess patient needs, develop and implement personalized care plans in collaboration with healthcare teams.

Estimated Salary: $45.00 per hour

Key Responsibilities:
  • Evaluate patients' rights and advanced directives to ensure comprehensive understanding.
  • Conduct ongoing assessments of patients' physical and psychosocial well-being.
  • Develop and implement tailored care plans in partnership with healthcare professionals.
  • Patient education and support services will be provided.
  • Care coordination with other healthcare professionals is essential.
Requirements:
  • Diploma, Associate or Bachelor of Science in Nursing degree is mandatory.
  • Licensure as a Registered Nurse in the State of New Jersey is required.
  • A minimum one-year adult, pediatric or medical/surgical clinical experience is preferred.

This position demands exceptional nursing assessment and documentation skills, as well as strong interpersonal and communication abilities.
