Job Summary

This position oversees the development, implementation, and maintenance of VAD training/education for patients and staff. The successful candidate will coordinate and manage the patient care of this high-risk patient population throughout the continuum to optimize the patient's clinical, functional, and psycho-social status. This role will provide the communication link for coordination of care among the multidisciplinary health care team involved with this patient type. The Nurse Coordinator will work with the NP coordinators in orchestrating a performance improvement program that focuses on continually improving the outcomes of these patients. This position will provide leadership and mentorship to the disciplines needed to support this patient population through application of experiential skills. The Nurse Coordinator will facilitate participation in cardiac/CT surgery-related clinical research trials. Patient care includes care to patients in the following age groups: adolescent adult (19 through 64 years) and geriatric (65 years and above), though primary age group will be geriatric. All team members are expected to be knowledgeable and compliant with Prisma Health's values of compassion, dignity, excellence, integrity and teamwork.

Key Responsibilities

  • Coordinates the care of the high-risk ventricular assist device (VAD) patient population across the continuum in collaboration with CT Surgeons, Heart Failure Cardiologists, NP coordinators, and other members of the multidisciplinary team. This involves advanced assessment, planning and coordination of care and education, and the monitoring and evaluation of performance improvement activities and outcomes.
  • Provides clinical consultation and education related to the management and care of the VAD devices for staff, physicians, and other disciplines who may have contact or care responsibilities for this population and facilitates the access and utilization of community resources and organizations to assist in patient's recovery.
  • Develops a therapeutic relationship with the VAD patient and family and maximizes the patient's participation and control in their own recovery while serving as the liaison and communication link between VAD patients and their families with the primary care providers, specialists, community organizations, patient care services and volunteer programs.
  • Teaches and provides reinforcement for VAD patients and their families on how to care for themselves and provide feedback on their progress. Provides education to acute care staff (ED, OR, ICU, cardiac surgery step-down).
  • Uses leadership skills such as time management and coordinating meeting multiple patient family needs and requests, meeting physician and team member expectations, setting priorities, networking , maintaining expertise and keeping current with health care trends.
  • Coordinates outpatient care of the VAD program in the clinic setting.
  • Shared on call requirement.
  • Plays an integral role in preparing and maintaining Joint Commission Accreditation to include but not limited to surveys, leading weekly multidisciplinary team meetings and database maintenance.
  • Collaborates with Heart Failure multidisciplinary team, providing surgical and VAD consultation in deciding optimal care decisions for this complex population.
  • Collaborates and assists with daily activities in the Heart Failure Program.
