Case Manager Project Based Housing Specialist

Step Up is seeking a skilled Case Manager to join our team in providing supportive services to tenants housed in our Permanent Supportive Housing Properties. As a Case Manager, you will be responsible for engaging with tenants, providing outreach and engagement, case management, and linkage to supportive services. You will work closely with our team to ensure that tenants receive the support they need to maintain stable housing and improve their overall well-being.
Key Responsibilities:- Coordinate lease up activities with Resident Manager and provide orientation to new tenants.
- Develop and implement individualized service plans for each tenant, guiding the services delivered.
- Meet face-to-face with each tenant a minimum of once per week, more frequently if needed, to assess and document changes in tenant income, housing needs, mental health needs, physical health needs, and vocational needs.
- Manage a caseload of 20-40 tenants and track, chart, and document all services provided.
- Participate in the lease-up process, including entering vacancies into MyOrg/HMIS and receiving referrals through the Coordinated Entry System (CES) where applicable.
- Collaborate with Mental Health providers to address tenant issues and formulate intervention strategies that focus on housing retention.
- Attend regular team meetings and service coordination meetings.
- Bachelor's degree preferred, with a minimum of 2 years' experience working with the mentally ill population in a community or housed setting.
- Strong computer skills and ability to work effectively in an interdisciplinary team.
- Familiarity with and dedication to the Housing First and Harm Reduction models required.
- Ability to work as a part of a team and maintain a non-judgmental, transparent approach in all interactions.
