Home Health Case Manager

We are seeking a highly skilled Home Health Case Manager to join our team at Nascentia Health. As a case manager, you will play a critical role in providing direct patient care and coordinating overall home care services.
Responsibilities- Provide high-quality patient care and ensure safe delivery of home health services
- Develop and implement individualized plans of care based on client assessments and OASIS evaluations
- Coordinate with other healthcare professionals to ensure effective patient outcomes
- Manage client caseloads and maintain accurate documentation
- Current New York State registered professional nurse license (BSN preferred)
- Valid driver's license and auto insurance
- Ability to work independently in a fast-paced environment
- Excellent communication and organizational skills
- Competitive salary range: $70,131-$81,275 annually
- $15,000 sign-on bonus
