About the Role

We are seeking an experienced Nurse Care Coordinator to join our team. As a key member of our care team, you will play a critical role in ensuring that our patients receive high-quality, patient-centered care. Your expertise and compassion will help us deliver exceptional results and make a meaningful difference in the lives of our patients.

Responsibilities

  • Develop and implement individualized care plans for patients, working collaboratively with our interdisciplinary team to ensure seamless transitions and optimal outcomes.
  • Provide clinical guidance and support to our caregivers, ensuring they have the necessary resources and training to deliver excellent patient care.
  • Coordinate patient scheduling, appointments, and follow-up visits to ensure timely and efficient delivery of services.
  • Maintain accurate records and documentation of patient interactions, progress, and outcomes.
  • Collaborate with our clinical team to identify areas for process improvement and implement changes to enhance patient satisfaction and quality of care.

Requirements

  • Bachelor's degree in Nursing (BSN) preferred; at least 2 years of experience in a clinical management role, preferably in home health care.
  • Demonstrated expertise in nursing practices, policies, and procedures, including PDGM regulations and compliance standards.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including patients, families, caregivers, and healthcare professionals.

Compensation

This is a full-time position with an estimated annual salary range of $85,000 - $105,000, depending on experience and qualifications.
