Job Title: MDS Nurse Coordinator

Mount Joseph at Waterville is seeking a skilled MDS Nurse Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for conducting and coordinating the development and completion of the resident assessment in accordance with state and facility requirements.

Key Responsibilities:

  • Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines.
  • Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities.
  • Assist in the recruitment, interviewing, and selection of personnel for the community or department.
  • Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and facility policies.
  • Coordinate and oversee the entire RAI process, including completion of documentation as necessary to ensure that time frames are met by the interdisciplinary team.

Requirements:

  • Nursing degree from an accredited college or university, or be a graduate of an approved RN/LPN program.
  • Current, unencumbered, active license to practice as a Registered or Licensed Practical Nurse in this state.
  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • AANAC certification or obtain certification within 6 months of hire.

Working Conditions:

  • Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending, and lifting.
  • Must be able to push, pull, move, and or lift a minimum of 25 pounds to a minimum height of 3 1/2 feet and be able to push, pull, move, and or carry such weight a minimum distance of 10 feet.
