The MaineHealth Preble Street Learning Collaborative is a low-barrier clinic that addresses the needs and care of people experiencing homelessness through education, outreach, and walk-in medical services, case management, and other community resources.

CONNECT Outreach Van

The CONNECT Outreach Van is a grant-funded mobile health and harm reduction program that delivers trauma-informed treatment for psychiatric and substance use disorders to people with unstable housing in Greater Portland.

Harm Reduction Program Coordinator

The Harm Reduction Program Coordinator position will support growth and expansion of the CONNECT program and offers the opportunity to join a close and supportive interprofessional team that provides medical care, social work, and basic support for patients with unstable housing.

Key Responsibilities
  • Support day-to-day program operations of the CONNECT Outreach Van through administrative activities, community engagement, and direct patient support.
  • Manage Harm Reduction program activities, such as inventory and assembly of harm reduction kits and creation of harm reduction communication materials.
  • Manage patient referrals documentation in EPIC, as well as coordination and communication of referrals among program partners.
  • Support grant and programmatic data collection and reporting activities.
  • Support other tasks as assigned by the Program or Clinical Director, or as the needs of the grant change.
Requirements
  • Associates or Bachelor's Degree preferred.
  • One year of experience in healthcare or related work experience required.
  • Strong computer skills.
