We are a 120-bed not-for-profit nursing facility dedicated to delivering exceptional care and rehabilitation services. Our picturesque eight-acre campus offers sub-acute services, wound care, and respite services, catering to the needs of both long-term and short-term residents.

We strive to create a center of excellence for our specialty programs and offer high-quality care to the elderly and physically frail population in our community.

We are seeking a per diem Physical Therapist who embodies the passion and commitment required to deliver the highest level of care to our residents. The successful candidate will be available to work on Saturdays and Sundays as well as intermittent weekday coverage.

• Initial Assessments: Conduct comprehensive assessments to evaluate patient status, therapy plans, and goals.
• Treatment Planning: Develop and implement individualized treatment plans tailored to each patient's unique needs.
• Patient Education: Provide instructive guidance to patients, families, and team members on enhancing and facilitating care.
• Compliance: Ensure adherence to all regulatory criteria and maintain professional standards.
• Leadership: Offer expert clinical guidance and leadership to team members while promoting a culture of continuous learning and improvement.

• New York State Physical Therapy License: Current and active license required.
• Education: Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association.
• Experience: Minimum one year experience as a physical therapist.

We offer an estimated annual salary range of $85,000 to $110,000, commensurate with experience and qualifications.
