Job Title: Registered Nurse RN

Empath Health is seeking a skilled Registered Nurse RN to join our team. As a Registered Nurse RN, you will be responsible for providing high-quality patient care and working collaboratively with our healthcare team to deliver exceptional outcomes.

Key Responsibilities:
  • Perform comprehensive assessments of patients' needs, including OASIS assessments as required.
  • Develop and implement individualized plans of care, revising as necessary to ensure optimal patient outcomes.
  • Provide nursing services and treatments as indicated on the plan of care, promoting patient independence and self-care.
  • Counsel and educate patients and their families on disease management, prevention strategies, and self-care techniques.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
Requirements:
  • Graduate of an approved nursing program and current Florida RN license.
  • BSN or MSN preferred.
  • Minimum one year of experience providing direct patient care, with previous experience as a home health RN highly desirable.
