Join SolutionHealth as a Nurse Assistant - Home Health Care and provide high-quality care to patients in the comfort of their own homes. As a member of our care team, you will perform delegated nursing, homemaking, and other delegated duties for patients of all ages and families. You will also report and document patient/client needs and responses to the appropriate Case Manager and/or Clinical Manager.

Key Responsibilities:

  • Perform delegated nursing, homemaking, and other delegated duties for patient/clients of all ages and families in their home or other designated facility.
  • Report and document patient/client needs and responses to the appropriate Case Manager and/or Clinical Manager.
  • Provide care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families.
  • Attend a minimum of 12 hours of in-services per year.

Requirements:

  • Current Nursing Assistant License in the State of New Hampshire required.
  • High school diploma or equivalent preferred.
  • Six (6) months to one (1) year of experience in a related field desired.
