Outpatient Coordinator Opportunity

Keller Oaks is seeking a skilled Physical Therapist or Occupational Therapist to serve as our Outpatient Coordinator. As a key member of our team, you will be responsible for building and strengthening relationships with current and prospective outpatient referral sources. This will involve completing face-to-face meetings with physicians, office managers, referral coordinators, and individuals in the community to educate them on the services our facility offers and discuss patient care, referrals, and existing referrals.

Key Responsibilities:
  • Develop and maintain relationships with local healthcare providers to promote our outpatient therapy services.
  • Identify and market to local primary and specialty physicians to address their patients' outpatient therapy needs.
  • Collaborate with the Director of Rehab to coordinate community events, public relations campaigns, or specialty marketing campaigns.
  • Manage the outpatient therapy schedule to ensure seamless patient care and coordinate with other therapists to reschedule any missed visits.
  • Provide high-quality therapy services to long-term care, skilled in-patient, and outpatient patients within the facility and in their homes, group homes, and assisted living facilities.
Requirements:
  • Licensed Physical or Occupational Therapist with a strong passion for patient care.
  • Experience in marketing in healthcare is a plus, but not required.
  • Exceptional interpersonal skills, including the ability to quickly build rapport with healthcare professionals.
  • Outgoing and personable with a willingness to become involved in local community events.
  • Minimum of 1 year experience in SNF and/or home health.
Preferred Skills:
  • Strong communication and documentation skills.
  • Ability to work effectively in a team environment.
  • Knowledge of therapy practices and procedures.
  • High degree of credibility and integrity.
