**Job Overview**

About the Job

As a Hospice Social Worker, you will be responsible for upholding the Code of Ethical Conduct and fostering positive working relationships within the company, among all departments, and with external stakeholders.

You will provide preventive, educational, and evaluative and treatment services to meet the psychosocial needs of patients and their families, helping them improve, restore, and maintain their maximum level of coping. To achieve this, you will utilize community resources and the interdisciplinary team (IDT) effectively.

Key Responsibilities

  • Explain hospice services and Medicare benefits to patients and families, obtaining Informed Consent and Election of Benefits documents as needed.
  • Submit appropriate documentation and paperwork to facilities at the completion of patient visits, adhering to company policy timelines.
  • Participate as a member of the IDT, contributing to the development and implementation of the plan of care.
  • Provide psycho-social support and counseling services to patients and families, supporting the involvement of external counseling resources commensurate with patient/family needs and scope of practice.
  • Identify the need for intervention from other IDT members and effectively communicate patient and family needs to the team.
  • Collaborate with the IDT to coordinate psycho-social care and support for patients and families, ensuring appropriateness, continuity, and quality of care.
  • Maintain updated, comprehensive care plans that reflect current problems, goals, and interventions for patients.
  • Maintain respect for patients' environments and belief systems, remaining nondiscriminatory regarding age, race, religion, sex, sexual orientation, national origin, physical or mental disability, and other personal matters.
  • Complete initial and ongoing psycho-social assessments for patients, identifying and addressing concerns/issues related to financial means, insurance, living arrangements/placement for long-term care, caregiver stressors, pre-bereavement and anticipatory grief, and community resource needs.
  • Facilitate placement in long-term care when necessary and monitor adjustment.
  • Participate in care planning meetings at long-term care facilities.
  • Support and facilitate advance planning, including living will/POA documents, funeral arrangements, memorial services, and body/organ donation.
  • Perform other duties as assigned.

Education and Experience

The ideal candidate will possess a minimum of a Bachelor's degree in Social Work, Psychology, Sociology, or a related field, along with one year of social work experience in a healthcare setting. Licensure may be required by state rules. A Master's degree in Social Work with similar experience is highly preferred.

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentage.
  • Strong written and verbal communication skills, including the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
  • Ability to understand, read, write, and speak English.

Location and Salary
