Job Summary

We are seeking a highly skilled Registered Nurse to join our Anticoagulation Clinic team at Duly Health and Care. As an Anticoagulation Clinic Nurse, you will play a critical role in managing anticoagulation therapy for patients, ensuring their safe and effective treatment.

Key Responsibilities
  • Anticoagulation Management: Demonstrates expertise in assessing, dosing, and planning care for patients on anticoagulation medications, including performing fingerstick INRs and using established protocols to make dosing changes as necessary.
  • Education and Counseling: Provides initial education to patients and their families upon referral to the ACC and continues to educate them as needed, empowering them to be active participants in their care.
  • Office Responsibilities: Manages office responsibilities, including documentation, records, and patient outreach, such as processing prescription refill requests and completing outreach to patients who are overdue for testing.
Requirements
  • Education: Graduation from an accredited Registered Nursing program with current and valid Illinois RN license.
  • Experience: Minimum 2 years of experience in an acute care setting or specific anticoagulation management required.
  • Certifications: Anticoagulation Therapy Management Certification Course within 6 months of hire by National Certification Board for Anticoagulation Providers.
