Job Summary

The Intake Coordinator plays a vital role in ensuring a seamless transition from acute care to home care for our patients. As a liaison between referral sources and our Pharmacy, you will be responsible for establishing and maintaining relationships with referral sources, facilitating the intake process, and verifying health insurance coverage.

Key Responsibilities
  • Establish and maintain relationships with referral sources to receive and process intake information for potential patients.
  • Facilitate the intake process by procuring all necessary patient information to determine home infusion admission.
  • Identify participating payers and contractual pricing agreements.
  • Verify health insurance coverage using verification forms.
  • Coordinate with referral sources, nursing agencies, and pharmacy staff to ensure a smooth transition for patients.
  • Perform case management updates for changes in therapy and patient status.
  • Notify patients or approved patient advocates of acceptance or denial of referrals.
  • Complete all applicable sections of Intake and Confirmation forms, and enter into CPR+.
  • Negotiate pricing with case management under the direct supervision of Center Manager.
  • Assist with the creation and maintenance of Medicare CMNs.
  • Obtain and track authorizations.
  • Complete monthly re-verification of Medicaid recipients.
  • Document credit and cash co-payments made at location level.
  • Obtain clinical documentation necessary for billing.
  • Responsible for the intake of personal referrals.
  • Data-entry and updating patients' demographic information.
  • Insurance benefit verification and coordination with nursing and pharmacy staff.
  • Prepare and manage paperwork and documentation for billing and collections.
  • Resolve patient/payee issues in a timely manner.
