Hueman Risk Adjustment Solutions is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in San Joaquin County. This is a full-time opportunity, scheduled Monday through Friday from 8 am - 5 pm. The provider will work on-site at the clinic, performing an average of 8-10 scheduled exams per day. No on-call, nights, weekends, or walk-ins.

Responsibilities:
Nurse Practitioners will have the opportunity to interact with members in a one-on-one setting, supporting their comprehensive care team. Providers performing HRAs are not required to treat or prescribe.
- Perform comprehensive, non-invasive physical assessments.
- Review medical history and ensure accurate and timely documentation.
- Conduct medical reconciliation to ensure patient safety and compliance.
- Provide basic patient education on wellness and preventative healthcare measures.
Qualifications and Experience:
Nurse Practitioners must have a Master's Degree or commensurate experience and satisfactory completion of NP license. Active State NP license, Active National board certification, and Current BLS certification are also required. 12 months of experience as a paid advanced clinical provider is preferred.
