Geriatric Care Nurse

Eaton County Health & Rehabilitation Services seeks a dedicated and compassionate Geriatric Care Nurse to join our team in providing exceptional care to our residents.
Key Responsibilities:
- Supervise and direct the day-to-day nursing activities performed by nursing assistants, ensuring the highest standards of care and compliance with regulations.
- Participate in the admission, transfer, and discharge of residents, adhering to regulations, standards of care, and facility policies.
- Complete and follow through on physician's orders, maintaining accurate and timely charting duties in accordance with regulations and facility policies.
- Develop and maintain a good working rapport with residents, family members, and personnel, ensuring that nursing services can be adequately maintained.
- Rounds with physicians, notifying them of any changes and adhering to Federal and State Regulations, as well as facility policy.
- Provide nursing care to residents in accordance with their scope of practice, Federal and State Regulations, and facility policy.
- Meet with residents and/or family members as necessary, reporting problem areas to the Clinical Mentor and Social Mentor.
- Recommend equipment and supply needs to the Clinical Mentor/Social Mentor.
- Review care plans and cardex daily to ensure that appropriate care is being rendered, that goals, problems, approaches, and revisions are based on the residents' needs.
- Inform and/or update any changes that need to be made on the care plan to the Clinical Mentor.
- Assist the Clinical and Social Mentor in revising the MDS, including corresponding supplemental assessments for accurate MDS coding and implementation of Triggers and RAP's.
- Ensure residents' rights are maintained per facility policy, Federal and State Regulations.
- Any disagreement with a supervisor must not be conveyed to staff. The Neighborhood Nurse can expect the same from the supervisor.
Requirements:
- Genuine interest in geriatric work.
- Ability to cooperate with others and use tact and patience in dealing with residents, their families, and visitors.
- Flexibility to meet changing demands as necessitated by variations in the nursing home routine.
- Ability to adapt to frequent changes in routine.
- Competence in operating a mechanical lift.
Experience and Education:
- Experience as a general staff nurse, demonstrating administrative and supervisory ability.
- Graduation from an accredited school of nursing and current registration/licensing with the State Board of Nursing Examiners.
- Participation in continuing education seminars and workshops to maintain current licensing and in-house inservice workshops.
