Job Title: Social Worker, Care Manager

Job Summary:

As a Social Worker, Care Manager with the University of Maryland Medical System, you will play a vital role in identifying patients' needs and connecting them with essential resources. You will work closely with an interdisciplinary team to assess patients' social and medical histories, identify barriers to care, and develop safe care plans. Your expertise in social work practice models, such as problem-solving and crisis intervention, will enable you to provide psychosocial interventions and support patients in achieving their goals.

Key Responsibilities:

  • Conduct thorough assessments of patients' social and medical histories to identify needs and barriers to care.
  • Develop and implement care plans that address patients' social and medical needs, ensuring their safety and well-being.
  • Provide psychosocial interventions, including problem-solving, crisis intervention, and solution-focused care, to support patients in achieving their goals.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Monitor patients' progress, identifying those at risk and providing timely interventions to prevent adverse outcomes.
  • Document all work in a timely and accurate manner, adhering to regulatory standards and departmental protocols.

Qualifications:

Education, Experience, and Qualifications:

  • Bachelor's degree in Social Work (BSW) from a Council on Social Work Education (CSWE)-accredited program.
  • Licensure as a Licensed Bachelor Social Worker (LBSW) from the Maryland Board of Social Work Examiners.
  • Preferred: Master's degree in Social Work (MSW) and licensure as a Licensed Master Social Worker (LMSW).

Knowledge, Skills, and Abilities:

  • Basic knowledge of social work practice models and theories.
  • Effective communication and interpersonal skills, with the ability to work with diverse populations.
  • Ability to prioritize tasks, manage time effectively, and adapt to changing environments.
  • Strong problem-solving and decision-making skills, with the ability to make decisions consistent with current policies and procedures.
