Childrens Case Manager

Job Summary:
The Children's Case Manager (CCM) at Assistance Plus provides professional case management services to eligible clients. This role involves advocating for clients to secure necessary services and coordinating all services deemed necessary to meet client needs.
The CCM will work effectively within an interdisciplinary team, demonstrating sensitivity towards others and a strong ability to work with children and adolescents. This position requires flexibility to work varying hours to meet client demand and may involve travel within a designated area.
Key Responsibilities:
- Provide professional case management services to eligible clients
- Advocate for clients to secure necessary services
- Coordinate all services deemed necessary to meet client needs
- Work effectively within an interdisciplinary team
- Travel within a designated area as required
Requirements:
- A minimum of one year relevant life/work experience working with children with a Serious Emotional Disturbance
- Must be 18 years of age or older
- Intermediate level computer skills
- A Bachelor's Degree from an accredited four-year institution of higher learning, with specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or closely related field
