This is an in-office position located in Sandy, Utah. The successful candidate will be responsible for ensuring clients receive high-quality care by assessing their needs and developing personalized treatment plans.

1. The Case Manager will accomplish this by conducting thorough intake interviews, reviewing therapist evaluations, and determining the need for therapeutic medical, psycho-social, and psychiatric evaluations.
2. They will also admit new clients by reviewing records and applications, and conducting orientations and assessments.

Scheduling clients with Addiction Psychiatrist and establishing treatment programs are crucial aspects of this role. The Case Manager must maintain accurate records by reviewing case notes, logging events, and progress.

1. Maintaining open communication with clients' clinician teams through regular coordination meetings and evaluations is essential.
2. The Case Manager will prepare clients for discharge by reviewing and amplifying discharge plans, coordinating requirements, and providing resources.

• Degree: High School Diploma or equivalent
• Experience: 3+ years customer service experience, preferably in human services
